Say "Bye Bye" To Your Thighs! - CoolSculpting FDA Cleared for Thigh Treatment

CoolSculpting FDA Cleared for Thigh Treatment

(see below for Before and After photos)

By now you have likely heard about CoolScupting, if not - check out our last blog post for a quick refresher!

To-date, over 1 million CoolSculpting treatments have been performed worldwide and CoolSculpting maintains the highest satisfaction rating on RealSelf.com among similar non-invasive, non-surgical technologies.

As Rochester's first 'CoolSculpting Certified' practice, we have performed thousands of treatments on hundreds of satisfied patients, which is why we are excited to offer treatment for the inner and outer thigh.

The new CoolSmooth thigh applicator is now FDA cleared for the treatment of stubborn fat on the outer thigh, while the CoolFit applicator can be used to treat the inner thigh!

"The CoolFit applicator features a flat vacuum applicator cup and 38% percent larger cooling plates than the other vacuum applicators. This allows physicians to treat longer, harder to reach areas such as the inner thigh. As a part of the clinical study submitted to the FDA, 84% of patients noticed visible fat reduction after just one treatment and 93% of patients were satisfied with their results. In addition, 90% of patients would repeat the procedure."1

Dr Lesley Loss is a local expert in the field of CoolSculpting, having presented at national meetings on new and unique applications of the CoolScupting technology in treating her patients.  She offers patients a broad range of applicators that CoolSculpting has to offer to meet each individual patient's needs.   She believes in having one of our licensed medical providers (either MD or NP) perform both the consultation and procedure  to insure proper candidate selection and accurate application of the device to yield the best possible results, which is why our satisfaction rating is over 95%!

The good news?

2/3 - is the number of people who reported improvement in social well-being and outlook after initiating appropriate medical therapy.

Last July we posted tips about managing rosacea.

Here we have identified some patient and provider, non-medication favorites for helping reduce redness.

JUVÉDERM VOLUMA™ XC

What is JUVÉDERM VOLUMA™ XC ?
JUVÉDERM VOLUMA™ XC is a dermal filler (injectable) that was recently FDA approved to temporarily correct age-related volume loss in the cheek area.  It is the first and only filler approved for this indication!

JUVÉDERM VOLUMA™ XC is comprised of hyaluronic acid, which is naturally found collagen and gives the skin its youthful, plump appearance.  Although regular JUVÉDERM is made of hyaluronic acid, VOLUMA is formulated using a VYCROSS™ technology that gives it the ability to produce more lift.   It can restore facial volume instead of just filling wrinkles around the nose and mouth.

The results are visible immediately after injection and the results can last up to two years!  In just 15-30 minutes, you can have a more youthful appearance!

Who is a candidate for JUVÉDERM VOLUMA™ XC?
The ideal candidate for VOLUMA is a patient with age-related facial volume loss of the cheek area.   Our providers can help you determine if VOLUMA is right for you.

How is the JUVÉDERM VOLUMA™ XC treatment performed?
JUVÉDERM VOLUMA™ XC is applied along the cheek bone area with a small needle.  Topical numbing cream can be applied before the treatment and ice can also be used to minimize discomfort.  The product also contains lidocaine, which decreased discomfort with each injection.

Is the JUVÉDERM VOLUMA™ XC painful?
Only mild discomfort is experienced with JUVÉDERM VOLUMA™ XC treatment.  Patients report slight pinch and/or pressure from injections.  The product contains lidocaine for comfort and a topical numbing cream can be applied in advance if needed.

Who performs the JUVÉDERM VOLUMA™ XC treatment?
Our board-certified physicians and nurse practitioners perform the procedure.  They have received extensive training in the art and science of injectable procedures.

How long does a JUVÉDERM VOLUMA™ XC treatment take?
A treatment takes between 15-30 minutes in the office.  The results are immediately visible.

Are there any side effects?
JUVÉDERM VOLUMA™ XC is approved by the FDA and is considered safe.  Possible short term side effects are injection site tenderness, redness, bruising, pain, itching and small nodules under the skin.

Can I return to my usual activities after treatment?
Patients can return to work immediately after the treatment.  We usually recommend no exercise for 24 hours after treatment.  We also suggest you avoid anything that could potentially manipulate the product (such as facial massage) for two weeks.

How long does JUVÉDERM VOLUMA™ XC last?
Clinical trials show volumizing results of JUVÉDERM VOLUMA™ XC  last up to 18-24 months.

Chapped and Cracked Hands - 7 Helpful Tips

The painful slender (or deep) cracks and fissures that many of us get on our hands are often exacerbated come winter time because of dryness, reduced atmospheric humidity, contraction of the skin due to cold, and extremes of weather.  If you have been suffering from this type of condition, you are not alone – hand dermatitis may affect as much as 10% of the population [Diepgen TL et al, Contact Derm 2007].  It is important to rule out an underlying allergic cause (allergic contact dermatitis or latex allergy), as well as an irritant cause (irritant contact dermatitis).  Yet, even in the absence of these types of conditions, many people still suffer from cracked hands and fingers.

PREVENTION:
As Benjamin Franklin wisely said: “An ounce of prevention is worth a pound of cure.”  Success in treating these painful and frustrating lesions requires a multi-step approach and diligence both in treating the cracks but ongoing adherence to the regimen to prevent new cracks and fissures.

1)   oral hydration:  while it is not known how much water a human should consume in a day (the 8 glasses is un-validated scientifically), it is important to make sure you increase your water consumption during periods when your hands or feet are more cracked than usual as this can help in the healing process.

2)   humidity:  winter is a dry season in much of the country.  Increasing the humidity in your home or work place can help decrease the rate at which your skin loses moisture, effectively allowing your skin to retain more water, support elasticity and keep it supple.

3)   occlusive barrier protection:  while no one likes feeling goopy and sticky, the use of heavy barrier creams and ointments like Vaseline, Aquaphor, Cetaphil hand cream, CeraVe hand cream, etc are the best means for locking moisture into the skin so that it is not lost, thereby resulting in drying of the skin.  Re-application of these products is important (albeit frustrating) and should be done at least 2-3 times per day, and every time you plan to go outside into the cold without gloves on.  As a side note – even Crisco (vegetable shortening) is an effective barrier emollient.

4)   bathing/showering/hand washing:  avoid extremes of water temperature.  Take a lukewarm shower or bath and be sure to pat dry instead of rubbing.  Immediate application of a barrier emollient will lock in the moisture.  If your job or habits result in frequent hand washing, use of a gentle soap like Dove, avoidance of anti-bacterial soaps, and repeated application of emollients is helpful.  Wearing gloves for dishwashing can also help minimize water lost from the skin.

5)   nighttime hand coverings:  application of a thick barrier emollient (like Vaseline or aquaphor) and then wearing a pair of cotton gloves overnight offers an additional protection for the skin.  Rubber gloves cause the hands to get warm and because they do not ‘breathe’ this ultimately results in loss of of water from the tissues and dehydration.

TREATMENT (in ADDITION to PREVENTION):
Sometimes you might try your best and still develop a painful fissure or crack in the palms or fingers.  Seeking advice from your dermatologist may be necessary as there are a number of prescription creams and ointments that can help kick-start the healing process and reduce local inflammation that may be preventing those wounds from closing.

1)   Topical steroid creams:  these should be used at the direction of your prescribing doctor (dermatologist or primary care provider).  When dermatitis or fissuring is severe it may require temporary or in some instances prolonged use of these treatments.  The side effect profiles are generally good but be sure to understand the specifics of how frequently and how long to use the prescribed regimen.

2)   Wound closure:  there are some very expensive products like Dermabond (or equivalent) available on the market (http://www.enablemart.com/johnson-and-johnson-dermabond-single-use-vial?gclid=CImQrZbR37sCFQbl7AodD1oA7g), however, it is not really necessary to spend $20-$50 for a single use vial, nor is it practical when you have numerous cracks on the fingers/hands.  When used carefully, any clear superglue that can be found at a supermarket, drug store, or hardware store is very effective in sealing cracks and allowing healing to occur from the bottom-up.  It can be reapplied when it starts to wear off in a few days.

Failure to improve, signs of worsening (like redness, pain, or swelling), or any other concerns you have should prompt evaluation by your dermatologist.

Sclerotherapy

Sclerotherapy (Leg Vein Injections)

Sclerotherapy is a minimally invasive procedure done by your healthcare provider to treat uncomplicated spider veins and reticular veins.  The treatment involves the injection of a solution into the affected veins.

Sclerotherapy can be combined with laser treatment with our GentlMax Pro or Candela VBeam laser.

FAQs:

How does sclerotherapy work?
A solution is injected into the vein with a very tiny needle.  It works by irritating the endothelium of the vessels, eventually causing the vessels to collapse and disappear. There are a variety of solutions that can be used depending on the type of vessels being treated.

How long does a sclerotherapy treatment take?
A typical sclerotherapy session lasts 15-45 minutes.  Multiple injections are done in one session.

Are the injections painful?
The needles used are very small, so at most there is a small pinching sensation.  Injection of the solution may also cause some minor burning that will dissipate.

What do I need to do after a sclerotherapy treatment?
Compression stockings are put on immediately after the procedure. It is necessary to keep pressure on the vessels treated for best results. Your healthcare provider will recommend that you wear compression stockings for two weeks following treatment.

What are the most common side effects of sclerotherapy?
Sclerotherapy is a very safe and effective procedure.  It is common to experience mild redness, temporary itching and slight bruising after undergoing a sclerotherapy treatment.   Additionally, brown-tan discoloration may occur that can last a few months after treatment.

When is the best time for treatment?
Generally, the best time for vein treatment is during the fall and winter, so your legs will be looking their best by the following spring or summer.

How many treatments will I need?
The number of treatments needed differs from patient to patient and depends on the extent of spider veins present.   You will have (50-70%) reduction or lightening in blood vessles after one treatment, so usually you need two to three treatments.   This can be discussed at your consultation.

Botox for Crow's Feet!

On September 11, the U.S. Food and Drug Administration (FDA) approved a use of Botox® Cosmetic (onabotulinumtoxinA) for the temporary improvement in the appearance of moderate to severe lateral canthal lines, which are more commonly referred to as "crow's feet."  Physicians have used Botox® in this area for years "off-label," but the new FDA approval will offer consumers an additional level of confidence in the safety and efficacy of the product.

Botox® Cosmetic was first approved in 2002 for treatment of glabellar lines (frown lines between the eyebrows).  The FDA has also stated that both frown lines and crow’s feet can be treated at the same time.

The FDA approval is based on two clinical efficacy and safety studies:

“The studies enrolled 833 adult participants with moderate to severe lateral canthal lines who were randomly assigned to receive Botox or placebo. Results showed that those treated with Botox had greater improvement compared to placebo in the appearance of lateral canthal lines.

The most common adverse reaction associated with the use of Botox Cosmetic for treatment of lateral canthal lines is eyelid edema, a condition in which the eyelids are swollen and contain excessive fluid.” (1)

Botox® is one of our most popular noninvasive cosmetic procedures for both women and men.  It is a purified protein that works by blocking nerve impulses, allowing muscles to relax, effectively eliminating wrinkles and giving the face a rejuvenated look.  Using a small needle, tiny injections are made into the muscles that cause frown lines.  Within days you may see an improvement, but it can take two weeks to see the full effect.  Typically Botox® treatment lasts three to four months.

Dermatology Associates of Rochester is also a member of the Brilliant Distinctions® Program, which is a program Allergan (the company who makes Botox®) created to allow you to get exclusive offers on Botox® Cosmetic and their other products including the cosmetic filler, Juvederm®.
